Digital Medication Plan
What is the Medication Plan service?
The digital Medication Plan offers both doctors and patients a comprehensive and up-to-date overview of the medications a patient is taking. The entire management of a patient's medication plan is consolidated under the Medication Plan service. In the future, important over-the-counter medications and dietary supplements can be included in the Medication Plan.
What are the benefits of the Medication Plan?
- Increases medication safety
- Reduces duplication of medications
- Improves collaboration between specialists and institutions
- Saves time in gathering information about medications
- Better understanding about the use of medications for patients
In the future, patients will be able to access their Medication Plan through the Health Portal.
How advanced is the Medication Plan solution?
On December 4, 2023, the prescription medication scheme became available through the Health Specialist Portal (TJT). The development of the digital Medication Plan is currently underway and will be delivered to users in two phases. In the first phase, the core functionality of the Medication Plan will be created, reaching users by the end of 2024. Healthcare professionals authorized to prescribe medications will be able to add new prescription medications, modify dosing schedules, delete medications from the plan, and view the Medication Plan history. The ability to refer medication approvals to another healthcare professional will be developed. The second phase will allow the addition of over-the-counter medications and patient-reported medications to the plan. Medication entries can be commented on, and notifications related to the plan will be available in the TJT. The view of Medication Plan will be available to patients in the Health Portal.
The digital Medication Plan solution is the first service using the HL7 FHIR data exchange standard in the upTIS system. With this service, the entire management of the patient's medication plan (including the creation, modification, and cancellation of digital prescriptions) will be transferred to the Health Specialist Portal. After the completion of this development and a transition period, digital prescriptions for medications can only be issued through the Medication Plan service.
