When filling in the application, please use the file formats (pdf or odt) on the EHIF website, documents sent as links will not be accepted. When submitting an application electronically, it is important to save the application form first on a computer, then fill it in with the necessary data and save it as a new document. Please provide the application submitted electronically with digital signature. Adobe Acrobat Reader is required to view and print PDF forms. You can download it on the Adobe website.
